Project Proposal Form








After submitting this first proposal form, my project was approved with minor modifications. My feedback involved citing specific creative practitioners, refining the research question, and allocating more and/or less hours to certain activities in the schedule.
This was the final version of my proposal form I submitted, having made the changes suggested by my supervisor. There have been slight changes to how I want to exhibit the outcomes of my research, and these can be seen in the Project Progression section.
Ethics Form








Filling out the ethics form initially was straightforward, however I did make a few mistakes with the technicalities of not adding my full signature, as well as amending the proposed start date - as I would've needed to get approval before starting my research.
I received detailed feedback from the School of DCI Ethics Rep (see communications via email below) and took them into consideration when resubmitting the form. The helpful, suggested revisions have been highlighted in yellow.

Signed Consent Forms




Despite having my friends and course-mates as my participants, it was clear that I needed to create consent forms due to the nature of my research:

Collecting survey responses

These forms ensured that, in terms of ethics, every participant understood what would be captured and how their data would be used.
